Departure sheet: concept, purpose, nuances of filling out

The main purpose of the departure address sheet is to confirm the fact that the citizen no longer occupies his previous place of residence in the Russian Federation and has been deregistered there.

In this regard, many migrants have questions about whether it is necessary to show a document when registering at a new address, and whether it is lawful to refuse registration without it. It is worth outlining the range of main situations when a migrant needs to have a form in Form No. 7 with him.

Situations when a sheet is necessary

The main situations when it is necessary to take care of obtaining a departure certificate include the following:

  • the citizen leaves the place of temporary stay earlier than the period specified in the registration certificate. If he has issued a temporary registration in a hotel or sanatorium, then such a document is not needed when leaving home on time. But if for some reason you have to leave this place earlier, then the departure form must be filled out upon checkout;
  • the migrant will be de-registered on his own and wants to register for a new place of residence as soon as possible. The fact is that if, during the process of registering at a new address, the registration authority does not have information about deregistration at the old one, a request will be made to the migration departments at the previous place of registration. In order not to waste time on such communication between government agencies, you can provide a sheet of departure from your previous place of registration, which will confirm that you are no longer tied to other addresses;
  • It is planned to register foreign citizens for permanent residence in Russia. In this case, a certificate of departure from the migrant’s home country will be required.

Design features: information, rules, nuances

The departure slip is a standard approved form (Order No. 288 of the Federal Migration Service of Russia “On approval of the Administrative Regulations for the provision by the Federal Migration Service of state services for registration of citizens of the Russian Federation at the place of stay and at the place of residence within the Russian Federation” dated September 11, 2012).

The citizen’s personal data is entered into the columns on the first page of the document, namely:

  • Full Name;
  • Date and place of birth;
  • citizenship;
  • at what address it was registered (where it is leaving from);
  • information about the registration authority;
  • identity documents: passport, residence permit, birth certificate;
  • place of new registration (where it arrives).

An arrival certificate is issued for the new place of registration (form 2).

Personal information on the back side is filled in depending on the situation. In this case, the following are required for everyone:

  • clause 12 “Where did you come from”: the data from clause 8 of the front side is duplicated;
  • clause 16 “The document was compiled”: the dates of the document’s preparation and the applicant’s signature are indicated;
  • clause 17 “Deregistration formalized”: the date of deregistration is noted and the signature of the official responsible for the procedure is affixed.

If necessary, information is provided on the reverse side that the person is moving within the same locality or has previously changed his last name / other personal data (for example, after marriage).

The form must be filled out in two copies. The purpose of the departure certificate is to certify the fact of discharge from a specific address within the country. At the same time, many of its points are duplicated in a document such as a sheet of statistical accounting of disposals, form 12c. It is filled out by citizens who travel outside the state. The document is used to calculate migration data.

Important reminder: to avoid mistakes, you should search for the required forms by numbers (for example, No. 7 or 12c), and not just by name.

The issue of filling out documents by foreigners is especially relevant. Some do not know Russian very well, others do not understand what to write in which section of the form. Departure sheet: concept

So, what is a departure slip and where can I get it? The main purpose of this paper is to confirm the fact that this citizen no longer resides at his previous place of residence in the territory of the Russian Federation and has been deregistered.

A departure slip is an official document that every citizen leaving their permanent place of residence must receive. The document will need to be presented at the new place of residence to obtain registration.

You can obtain this document at the passport office or at another department that provides this service; for this you only need to fill out a special application. You can ask the staff for a sample form.

Why do you need a departure address sheet? This sheet has a statistical registration coupon; the data obtained is used in the statistical developments of Rosstat.

Based on the information received, a statistical report is prepared, which reflects information about which cities and regions the population is moving from and to which part of the country these citizens are moving.

Is it necessary to have a departure slip?

The Ministry of Internal Affairs deals with migration issues in Russia. The procedure for deregistration at the place of residence is established by Order of the Federal Migration Service of Russia dated No. 288. Thus, clause 31 of the Order establishes the right of citizens upon arrival to a new place of residence to provide Form No. 7, issued at the previous address of residence. It must be issued by the person responsible for receiving and transmitting documents to the regulatory authorities. But the presence of such a certificate is not necessary, and refusal of registration in its absence is illegal. Therefore, if you do not have this document, the migration service at your new place of residence will make two notes in your passport: about departure and about arrival.

Why do you need a departure slip and where to get it?

The departure address sheet (form 7) is included in the list of documents developed by the Main Directorate for Migration of the Ministry of Internal Affairs of Russia (formerly the Federal Migration Service) for the registration procedure at a person’s place of residence/stay in the Russian Federation.

Registration is carried out by the Main Directorate for Migration of the Ministry of Internal Affairs of Russia. To obtain an extract, you must contact the local migration office or any of the government bodies that provide intermediary services between the citizen and the migration service:

  • passport office at the housing office;
  • MFC;
  • State Services portal.

Important ! Only during the self-discharge procedure does a person receive a departure address sheet.

When a minor is discharged, all documents are drawn up for him by his parents, adoptive parents or legal guardians, subject to permission from the guardianship authorities.

At the new place of residence, a person submits his passport for registration along with the departure address sheet.

Why is it needed?

If a citizen stays on the territory of the Russian Federation for more than 90 days, he must obtain temporary registration.
In this case, the location can be anything, from rented apartments and hotels to sanatoriums and hospitals. To achieve this, the Federal Migration Service has established a procedure that takes into account all actions of citizens and representatives of the migration service. If a person is registered in the same locality, but lives at a different address, he also requires temporary registration. One of the documents required for registration is the arrival address sheet Form 2 (you will find the form below). It was established by FMS Order No. 288 of September 11, 2012 and is needed to simplify the process if the duration of stay is known and determined. The period of stay at the place of temporary registration must be at least six months. The arrival sheet is included in the list of documents required for temporary registration and is issued at the passport office (OUFMS). Since it is a document for the current registration of migrants, it is transferred to Rosstat.

IMPORTANT!

A static arrival record sheet can also be confused with an address sheet. The main difference between them is that the latter is filled out and attached to the documents when a person plans to temporarily register at the place of stay for a period of more than 9 months.

Is it necessary to show a departure sheet when registering?

Let us remind you once again whether you need a departure slip when registering at a new address. The law does not require the presence of this form when submitting documents for registration.

A citizen can visit the department only once - when registering at a new address. In this case, a request for an extract is filled out on the back of the application for registration. And the migration authorities themselves transmit information about the old place of residence.

Thus, both stamps are affixed to the passport in one place - both the registration and the extract.

Therefore, the answer to the question whether it is possible to register without a departure slip has a positive answer. And if representatives of migration authorities refuse to register a citizen only on the basis of the absence of this form, their actions are unlawful.

Is it necessary to show a departure slip when registering?

You can register at your new place of residence without any problems, even if you don’t have a departure certificate . The law does not require the availability of this document, but it can significantly speed up this process.

If you have the paper, the applicant will only need to contact the Migration Department once to complete the registration.

If the applicant does not have a departure certificate, he will have to write down a request for deregistration at his previous place of residence on the back of the application for registration at the new place of registration. In this case, the authorized body will independently carry out the entire procedure.

As soon as all procedures are completed, the applicant will be given both stamps in one place, one about departure from the place of residence, the other about arrival.

But what to do if, when leaving the previous place of registration, the departure slip was taken, and then, in the bustle of moving, packing and unpacking things, this paper was lost?

Since the departure slip does not have to be presented at the new place of registration, no non-standard measures will have to be taken. All you need to do is contact the Federal Migration Service and register at your new place of registration.

The form of the questionnaire is established by Order No. 288 and is known as form No. 7. (departure address sheet, form 7) can be found at the end of the article. There are usually no difficulties when filling out the questionnaire. It consists of 17 points.

The front side contains the following information:

  1. Name (in full).
  2. Surname.
  3. Middle name (if available).
  4. Date of birth in the format "DD, MM, YYYY". Important: the month is in words, and the date and year are in numbers.
  5. Floor.
  6. The place of birth is indicated as recorded in the passport or birth certificate.
  7. Citizenship.
  8. Registration information at the address of residence or stay. The date is indicated first, then the full address.
  9. Regulatory accounting authority: indicate the name and code of the unit to which the application and form No. 7 are submitted.
  10. Identity document indicating the type, series, number, date and authority that issued it.
  11. New address.

The reverse side contains paragraphs 12–17, of which only the twelfth is mandatory, the rest are filled in as necessary:

  1. Former residence address. Repeats information from paragraph 8.
  2. New residential address if the move took place within the same locality.
  3. If there are changes to your full name. and other information (date of birth, gender), they will need to be indicated in this paragraph.
  4. We indicate why the personal data has changed.
  5. The person filling out the form puts the date of completion and signs it.
  6. The official responsible for the correct completion of the form puts the date and signature. This will mean that all the information presented in the document is verified and reliable, and the sheet itself is formatted correctly.

Important: the document must be completed in two copies.

So, the documents have been completed and submitted. The person responsible for their reception and transfer to the registration authorities, within 3 days, transfers the passport, application and departure address sheet to the territorial body of the Ministry of Internal Affairs of Russia. Based on the information received, the registration authority carries out deregistration at the previous address within 3 days, and within 1 business day information about this should appear in the automated database. After this, you can pick up a second copy of Form No. 7 and a passport with a mark on deregistration.

You must register within 7 days from the date of arrival at your new place of residence.

To register for permanent residence in Russia, citizens of countries under a simplified procedure also need to draw up such a document (in particular, citizens of the Republic of Belarus).

Features of the design of the departure slip

How to fill out the departure address sheet according to form No. 7? An application for deregistration and receipt of a departure certificate in 2021 is filled out using the standard form of Form No. 7.

This form was approved by Order No. 288 of September 11, 2012 of the Federal Migration Service of Russia (“On approval of the Administrative Regulations for the provision by the Federal Migration Service of the state service for registration of citizens of the Russian Federation at the place of stay and at the place of residence within the Russian Federation”).

Filling out the document is not difficult, since all the required points are numbered and deciphered:

  • The first thing to do when filling out the paper is to indicate the full name of the applicant;
  • you also need to indicate personal data: date and place of birth, citizenship, at what address the citizen was registered before departure;
  • you need to indicate the registration authority that issued the registration document;
  • identification document of the applicant - if the person is an adult - a passport, if not - a birth certificate, this may be a residence permit;
  • You will need to indicate the place where the applicant intends to leave for further residence.

If necessary, you will need to fill out the form on the back side:

  1. In paragraph 12, you will need to duplicate information about where the applicant came from.
  2. Item 16 – here you need to enter the date the document was drawn up and sign the applicant.
  3. Item 17 “Deregistration formalized” - we write the date of deregistration, in this paragraph the signature is placed by the specialist who deregisters the applicant and is responsible for carrying out this procedure.

If necessary, other personal data is indicated on the reverse side, for example, if there was a change of surname or other personal data, as well as information about if a person moves within the same locality.

This form must be filled out in two copies . Both papers, together with the passport and the application for deregistration, are transferred to the Main Department of Migration of the Ministry of Internal Affairs of the Russian Federation for further work. After completing the registration procedure, the applicant receives a ready-made document, registered with the signature of an official and the seal of the organization.

How to fill it out correctly

When deregistering on your own, you must write an application (sample) and fill out Form 7 in two identical copies. How to correctly fill out the Form 7 departure address sheet?

Important ! The application and Form 7 are filled out only on the basis of documents (general passport, birth certificate for minors, etc.) of the person who is being discharged from this living space.

There are 17 points on the departure sheet, of which 1 to 12 are required, which reflect the following information without abbreviations or errors:

  1. Name.
  2. Surname.
  3. Middle name (if available).
  4. Date of birth in the format “DD, MM, YYYY”: the month is written in words, the day and year – in numbers.
  5. Floor.
  6. Place of Birth.
  7. Citizenship.
  8. Where the person was registered at the place of residence/stay.
  9. Registration authority (indicating the department code).
  10. Identity document.
  11. Where does the individual go?
  12. Where did you come from?
  13. To be filled in if a person moves within the same locality.
  14. If a person changed his last name, the old last name and other information about this last name are indicated.
  15. Other reasons.
  16. The date the form was filled out and the signature of the person deregistering.
  17. Date of deregistration and signature of the official.

Points 13-15 are filled out as necessary.

The departure address sheet contains a block of machine-readable data for automated accounting. A sample of filling out the Form 7 departure address sheet is presented below.

The employee accepts the documents, checks the correctness and accuracy of completion.

Upon receipt of a passport with an extract stamp, one copy of the departure address sheet signed by an official is issued. The second copy remains with the accounting authorities. The departure slip is valid for 30 days. Upon arrival at your new place of residence, you must register within 7 days.

Filling procedure

The address sheet is filled out independently as follows:

  • in the first 3 paragraphs indicate the applicant’s full name only in the nominative case;
  • in column 4 enter the date of birth (for example, July 7, 1991);
  • in paragraph 5 indicate the gender of the person (male or female);
  • in column 6, enter your place of birth in the following sequence: country, region of the Russian Federation, city (for example, like this: Russian Federation, Belgorod region, Belgorod);
  • in subsection 7 indicate the presence of Russian citizenship;
  • in column 8 enter the date and place of registration;
  • in paragraph 9 indicate the government agency from the passport stamp with the registration address;
  • In column 10, enter passport data (number, series, at what time it was issued and by whom it was issued);
  • in subsection 11, the address of the previous place of residence must be indicated, indicating a specific region of the Russian Federation, district, city or other populated area, a specific street, apartment or private building, separate building (if any), apartment (for example, like this: Chelyabinsk region, the city of Magnitogorsk, Karl Marx street, 25, building 2, apartment 7).

On the second side of Form No. 2, fill out the following items:

  • Column 12 is filled in when receiving temporary registration in the previous city, but at a different new address;
  • in paragraph 13, data is entered on the change in the previous surname, name or patronymic after marriage or under other similar circumstances;
  • additional (important) information is entered in column 14;
  • in subsection 15 they put the date of execution of the document and a personal signature (for example, May 7, 2021).

The remaining points are completed by authorized employees of the Department of the Ministry of Internal Affairs of the Russian Federation.

Lost departure slip - what to do

Moving is a troublesome business. And in the turmoil of collecting things and organizing other things, documents may remain in the old apartment. Citizens who are faced with similar unpleasant situations are wondering what to do if the departure slip is lost.

Considering the fact that when registering, it is not necessary to have this form of document with you, the answer is simple: you don’t need to do anything. You just need to fill out a request for deregistration at your previous address in the application for a new registration.

What to do if you have lost your departure slip?

To register at a new place of residence, a package of necessary documents is provided. The departure address sheet is not considered mandatory in this package. Its presence is desirable, but not required.

Important ! If a person does not have Form 7 (for any reason, including loss), the employee does not have the right to refuse to accept documents for registration at a new place of residence.

A citizen applying for registration writes a statement about the reasons for the absence of Form 7.

If you still need the lost departure address sheet (Form 7) for other purposes, you can restore it at the place of discharge - write an application and receive a duplicate.

What is an arrival address sheet

The arrival address sheet (hereinafter referred to as the Address sheet) is considered to be the official certificate “Form No. 2”, which confirms the fact of a citizen’s move to another city or to a new place of residence. When receiving a temporary residence permit, such a document must be submitted to the regional Department of the Ministry of Internal Affairs of the Russian Federation.
This certificate contains information about the previous city, the new city and the period of stay at a specific new place of residence. Important! A sample of Form No. 2 is taken from Order No. 288 of the Federal Migration Service of the Russian Federation dated September 11, 2012. The period of stay of a citizen of the Russian Federation or an immigrant at the place of temporary registration is at least 6 months. After being accepted for registration, such a certificate is immediately transferred to Rosstat.

A statistical arrival sheet is often confused with such a certificate. The main difference between them is that the latter is issued upon receipt of temporary registration at a specific place of stay for a period of more than 9 months.

Departure acts

When changing his place of residence, a Russian can receive a certificate of departure at his previous address of residence. If such a document is lost, the person notifies in writing of the circumstances due to which the document cannot be presented. This is done at the newly chosen place of residence.

Important! However, the absence of an act of departure does not become an obstacle to the submission of application documents for the chosen place of residence and in subsequent procedures.

People become removed from the account:

  1. In a place of short-term stay in housing not intended for their long-term residence - at the end of the period of such stay.
  2. In hotels, hospitals, tourist centers and other accommodation organizations for guests - upon their departure.

There is no need to apply for de-registration at the place of short-term stay from an individual and to draw up an act of departure using form No. 7. An exception is if persons registered at the place of short-term stay intend to leave there before the end of the time specified by them earlier.

When discharging from medical institutions persons registered in them at the place of short-term stay, an authorized employee of this institution, within 3 days from the date of discharge, draws up departure certificates for them.

Number of copies - 2:

  • The 1st document is sent to the local branch of the address bureau;
  • 2nd - transferred to the regulatory authority.

If the FMS refuses to establish the status of forced migrants on the basis of messages from the FMS, at the end of 3 months from the date of registration of such persons at the place of stay in places of temporary settlement, the regulatory authorities remove them from the registration system in the manner established by the Administrative Regulations.

A mark on the removal of citizens from registration at the place of residence is placed on 2 copies of departure slips of form No. 7 (Appendix 7 to the Regulations), which are issued to citizens along with an identity card.

A similar document is drawn up in 2 or 3 copies:

  • 1st - for an individual applicant;
  • 2nd - for the local (territorial) body of Rosstat;
  • 3rd - for the Rosstat Office for a specific region of the Russian Federation.

Algorithm for discharge from residential premises

In the correct situation, at the time of discharge from residential premises, each citizen is obliged to record this fact. Where can I get a departure slip? To do this, you need to contact your local migration registration authorities .

Previously, this function was performed by passport offices, today this body has already been simplified in many regions, so for this paper you will have to contact the relevant government bodies under the jurisdiction of the territorial branch of the Migration Department of the Ministry of Internal Affairs of the Russian Federation.

There is another body authorized to perform this service - the MFC. Therefore, if you wish, you can go there.

As mentioned above, the applicant must fill out form No. 7 in two copies; copies of all necessary papers are attached to the package of documents.

According to the regulations, it takes three days to consider the application . If you order a service through the MFC, you will have to wait a little longer, since the MFC itself does not provide this service, but only collects applications. After which all papers are transferred for work to the Migration Department.

Upon expiration of the agreed period, the applicant receives ready-made papers, which he will have to present at the place of next registration. He also receives a passport with a note indicating deregistration.

The deregistration procedure is completely free and does not require payment of any state fees.

What is needed to obtain a departure certificate

Having understood the peculiarities of filling out a departure form, it is logical to discuss the algorithm of actions for citizens to complete it. To confirm the legal fact of the extract, you need to contact the relevant government authorities with a certain package of documents.

What documents need to be provided

For those who are interested in how to obtain a sheet of form No. 7, the following list of documents is provided:

  • an identity card, because almost all the data on the departure form is filled out on its basis: for citizens of the Russian Federation - a passport;
  • if a child is being discharged, a birth certificate;
  • for a temporary resident - a residence permit;
  • for a foreigner - a passport of his home country;
  • application for deregistration.
  • The forms for minors include the arrival and departure addresses of their parents.

    Authorized bodies

    To obtain a departure certificate, a citizen needs to contact the relevant government agencies. In the past, this was done by passport offices. Currently, issues of registration of citizens are under the jurisdiction of the territorial branches of the Directorate for Migration Issues of the Ministry of Internal Affairs of the Russian Federation.

    Price and processing time

    The deadline for issuing a departure certificate is no more than 3 days. The applicant is returned one copy of completed form No. 7 and a passport with a note about the extract.

    The procedure for deregistration is free; administrative regulations do not provide for a state fee for issuing a departure certificate and affixing a deregistration stamp in the passport.
